4、企业组织机构代码证复印件一份；

5、企业近期财务审计报表；

6、企业法人、项目负责人、技术人员简历及从业资格证书；

7、拥有主要设备及工具明细表；

8、企业主要工程业绩和主要合作企、事业单位的满意证书1-2份；

以上所有证明材料均需加盖企业公章。

第十七条 参与评定的企业在递交申请材料后，三个月内由中国舞台美术学会的专家评审小组按资质等级评定标准，通报评定结果。次未通过的企业应在递交申请材料的6个月后，按中国舞台美术学会专家评审小组提供的意见书，整改后方可再次申报。

第十八条 本评定办法由中国舞台美术学会解释。
